The Respiratory System

The respiratory system is a complex network of organs that enables gas exchange between your body and the environment. This comprehensive course covers upper and lower respiratory structures, from the nose and pharynx to the alveoli and pleura, along with breathing mechanics, gas transport, and respiratory control mechanisms. 1. Upper Respiratory System Anatomy: The nose and nasal cavity serve as the primary air entry point, featuring specialized structures like conchae and meatuses that increase surface area for air conditioning. Paranasal sinuses in the frontal, ethmoid, sphenoid, and maxillary bones produce mucus and may help warm inhaled air. The pharynx, divided into nasopharynx, oropharynx, and laryngopharynx, connects the nasal cavity to the larynx while housing tonsils that trap pathogens. These structures work together to filter, warm, and humidify incoming air before it reaches the lungs.

2. Lower Respiratory System Organization: The larynx contains critical cartilages including the thyroid (Adam's apple), cricoid, and epiglottis, plus vocal cords for speech production. The trachea extends from the larynx to the carina, where it bifurcates into right and left primary bronchi. The bronchial tree continues branching into lobar bronchi (serving each lung lobe), segmental bronchi (serving bronchopulmonary segments), and progressively smaller bronchioles ending at terminal bronchioles. This hierarchical branching efficiently distributes air throughout both lungs while maintaining structural support through cartilaginous rings.

3. Alveolar Structure and Gas Exchange: Terminal bronchioles branch into respiratory bronchioles, then alveolar ducts, and finally alveolar sacs containing approximately 150 million alveoli per lung. Type I pneumocytes facilitate gas exchange across the respiratory membrane, while type II pneumocytes produce surfactant to prevent alveolar collapse. Alveolar macrophages remove inhaled particles and pathogens. The extensive capillary network surrounding each alveolus creates an ideal environment for oxygen and carbon dioxide exchange, with the respiratory membrane being only 0.5 micrometers thick to optimize diffusion.

4. Lung Anatomy and Pleural Organization: The right lung has three lobes (superior, middle, inferior) separated by horizontal and oblique fissures, while the left lung has two lobes (superior, inferior) with a cardiac notch accommodating the heart. Each lung contains bronchopulmonary segments that function as independent units. The pleural membranes consist of visceral pleura covering lung surfaces and parietal pleura lining the thoracic cavity. Pleural fluid in the pleural cavity reduces friction during breathing and creates surface tension that helps lungs adhere to the chest wall during expansion.

5. Breathing Mechanics and Pressure Relationships: Ventilation depends on pressure gradients created by thoracic cavity volume changes. During inspiration, diaphragm contraction and external intercostal muscle activity increase thoracic dimensions, decreasing intrapulmonary pressure below atmospheric pressure and drawing air into the lungs. Expiration during quiet breathing relies on elastic recoil of stretched lung tissues and relaxation of inspiratory muscles. Intrapleural pressure remains negative (approximately -4 mmHg) due to opposing forces between lung elastic recoil and chest wall elasticity, preventing lung collapse and maintaining transpulmonary pressure gradients.

6. Gas Transport and Exchange Physiology: External respiration involves oxygen diffusion from alveolar air (PO₂ = 105 mmHg) into pulmonary capillary blood (PO₂ = 40 mmHg), while carbon dioxide moves from blood (PCO₂ = 45 mmHg) to alveolar air (PCO₂ = 40 mmHg). Internal respiration occurs at tissue capillaries where oxygen moves into metabolically active cells and carbon dioxide enters the bloodstream. Hemoglobin carries 98.5% of oxygen in arterial blood, with binding affinity influenced by temperature, pH (Bohr effect), and 2,3-BPG levels. Carbon dioxide travels as dissolved gas (7-10%), carbaminohemoglobin (20%), and bicarbonate ions (70%) via the chloride shift mechanism in red blood cells.

7. Respiratory Control Mechanisms: The medullary respiratory center in the brainstem contains the dorsal respiratory group (DRG) controlling inspiratory muscles and the ventral respiratory group (VRG) regulating breathing rhythm through the pre-Bötzinger complex. The pontine respiratory center fine-tunes breathing patterns during activities like speech and exercise. Central chemoreceptors in the medulla respond primarily to CO₂ and H⁺ changes, while peripheral chemoreceptors in the aortic arch and carotid bodies detect CO₂, H⁺, and O₂ levels. Respiratory rate increases significantly only when arterial PO₂ falls below 60 mmHg, creating feedback mechanisms that restore normal breathing patterns.
